An Advance Directive, commonly referred to as a Living Will , enables you to specify in advance the types of medical treatments and life-prolonging measures you want – or don’t want – in the event of a serious accident, illness, or end-of-life situation.  This is a very important document to have, as it speaks for you in case you are unable to speak for yourself – for example, if you were in a coma.  Your family and doctors will consult this document in the event that you are incapacitated and unable to make your own healthcare decisions.

Included in this document is a Healthcare Power of Attorney, which designates someone to make healthcare decisions on your behalf.  Also known as a Health Care Proxy or Medical Power of Attorney, this part of the document allows you to name someone to make decisions about your medical care, including decisions about life support, if you can no longer speak for yourself.
